General Summary of Position Provides professional nursing care for a disease-specific subset of patients. Liaisons are expected to provide individualized assistance to patients entering the health care system. Functions to advance the clinical mission and goals of MedStar Health. Primary Duties and Responsibilities
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.Performs professional clinical services in the outpatient clinical setting including, but not limited to: patient assessment, communication between patient and provider, communication between MedStar Health and referring providers, identifying and decreasing barriers to care, and facilitate prompt access to a provider that is in alignment with patient needs.Collaborates and coordinates withs and all disciplines involved in the patient's care to assure timely progress toward the best clinical outcome in the most cost-effective manner.Facilitates the evaluation of patients by interfacing with referring providers, clinical research coordinators, scheduling coordinators, consultants, Insurance coordinators and social workers. Has primary responsibility for the planning, implementation and evaluation of this process.Coordinates the complex management and education of patients to ensure seamless integration with the healthcare team. Works closely with the scheduling team to ensure prompt facilitation of appointment(s) with providers.Monitors triage line for individual center/program.Screens patients for appropriate information. Instructs patients in the collection of samples and completion of tests. Performs patient telephone triage, assesses needs, and coordinates needs with medical staff.Maintains and reviews patient records, charts, and other pertinent information. Obtains and reviews consent forms, treatment plans, and side effects as appropriate. Enters preliminary patient history information into electronic medical records in advance of provider visits.Schedules in-services and education sessions with patients, families, and healthcare professionals. Identifies communication barriers, financial concerns, and cultural factors to overcome disparities in care.Identifies opportunities for patient participation in clinical trials whenever feasible. Communicates information to research coordinators and referring physicians as necessary to promote enrollment.Is responsible for creating a database/registry of all patients which will be used for follow-up, research, and or conferences. The registry will be updated and maintained on a regular basis.Coordinates the multidisciplinary conferences.Assumes other duties and responsibilities that are appropriate to the position and area. Participates in multi-disciplinary quality and service improvement teams.
Minimum Qualifications Education
- Bachelor's degree in Nursing (BSN) required and
- One year of relevant education may be substituted for one year of required work experience.
Experience
- 3-4 years Professional clinical experience required and
- 1-2 years Previous triage, or outpatient experience highly desirable preferred
Licenses and Certifications
- RN - Registered Nurse - State Licensure and/or Compact State Licensure Registered Nurse license in the District of Columbia, Maryland, and/or Virginia Upon Hire required and
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and
- Additional unit/specialty certifications may vary by department or business unit.
Knowledge, Skills, and Abilities
- Knowledge of professional nursing theory and practice to assess, plan, implement, and evaluate patient care.
- Knowledge of medical equipment and instruments to administer patient care.
- Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care.
- Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public.
- Ability to react calmly and effectively in emergency situations.
