This position provides leadership of data-driven patient safety initiatives. The Patient Safety Program Manager is responsible for working collaboratively with key leaders across the market and enterprise for the purpose of measurably improving patient safety and reducing patient harm/errors. This requires responsibility for performing Patient Safety Investigations and for supporting and performing patient safety event data analysis and activities that support UPH's enterprise safety initiatives. This position assists in the resolution of safety issues/concerns by facilitating root cause analysis and action planning with cross-functional teams, leading them through process improvement activities utilizing evidence-based quality improvement tools and methodologies.

Responsibilities

Lead patient safety strategy / Resource for patient safety

  • Lead the development of a comprehensive Patient Safety Plan that includes long-term objectives and realistic strategies.
  • Provides feedback to update the enterprise plan as needed and provides an annual assessment of plan completion.
  • Work closely with Risk Management, other market Patient Safety leaders, and UnityPoint Health Leadership, to conduct an annual assessment of patient risks and recommends patient safety goals.
  • Keep abreast of federal reporting regulations and guidelines related to patient safety.
  • Coordinate organizational response to regulatory standards related to patient safety including but not limited to DNV requirements.
  • Serves as Just Culture Coach and provides education as needed including during orientation.
  • Serves as a subject matter expert in patient safety science (ie Human Factors Analysis) and provides internal consultation and education/training as needed.
  • Facilitates prevention, identification and reporting of patient safety issues and problems, role-modeling nonpunitive approaches to improvement.

Clinical Event Identification & Investigation

  • Reviews daily RL report listings for potential level I - IV Safety Events and initiates appropriate notifications and initial review
  • Determines initial event classification level (Level I - IV)
  • Leads patient safety investigations including chart reviews, staff and provider interviews, collection/analysis of other relevant documents.
  • Identifies contributing factors such as staffing levels, environmental factors, human factors etc.

Root Cause Analysis Facilitation & Corrective Action Planning/Implementation

  • Leads and facilitates cross functional teams in the completion of RCAs for Level I - III Events and those identified as needing an RCA
  • Assures completed RCAs including the Action Plan are shared with the Critical Event Committee (CEC) and when appropriate the Enterprise CEC
  • Coordinates monitoring and action plan tracking for serious safety events and reports follow-up as appropriate.

Qualifications
  • Education: Bacherlor's Degree in Healthcare Related Field required. Master's degree preferred.
  • Experience:Minimum of five years of healthcare experience. Basic understanding of care processes provided in multiple sites of care. Prior healthcare leadership responsibility. Knowledge of rapid cycle improvement theory and human factor principles.
  • Licenses and Certifications: Certified Profession in Patient Safety (CPPS) preferred. Just Culture Certification preferred. Six Sigma Black Belt, Lean or Adaptive Design certification.
  • Other: Ability to communicate effectively and document information accurately. Effective interpersonal relationships and communication skills (verbal, non-verbal and written). Use of usual and customary equipment used to perform essential functions of the position.
