UC San Diego Health is seeking a dynamic and experienced Registered Nurse (RN) to lead our innovative UCSD at Home program. This position is housed within UC San Diego Health Population Health Services. This role oversees a multidisciplinary team including home-visiting providers, RN Case Managers, Social Workers, and a Medical Assistant Supervisor. The program functions as a virtual clinic, delivering high-touch care to high-risk seniors in their homes to promote safety, independence, and wellness.

MINIMUM QUALIFICATIONS
  • A Bachelor's or Master's Degree in Nursing.

  • Registered Nurse (RN) license issued by the State of California.

  • A minimum of five (5+) or more years of relevant experience.

  • Previous acute inpatient or ED RN experience.

  • Prior experience managing a fast paced clinic and/or a large team; or recent UCSD at Home experience with minimum of three (3) years in program.

  • Strong knowledge of case management, discharge planning, home care, and / or managed care.

  • Demonstrated ability to lead process improvement projects across departments, with strong communication and data skills. Ability to create standard work documents and program policies.

  • Strong written and verbal communications skills, with the ability to train and mentor subordinate staff, convey complex clinical and technical information in a clear and concise manner, and prepare and present a variety of reports, documentation, analyses, and project proposals.

PREFERRED QUALIFICATIONS
  • Knowledge of Epic and UC San Diego Health.

  • Two (2+) or more years of supervisory or management experience.

  • Experience supervising union-represented employees.

Pay Transparency Act

Annual Full Pay Range: $132,600 - $259,800 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $63.51 - $124.43

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
