The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Job Overview: The Patient Navigator for the Peer Recovery Program is responsible for effectively guiding patients with substance-use disorders through the healthcare system and resources needed to initiate and maintain recovery. The Patient Navigator will screen the individual for social determinants of health, determine the most appropriate services to address their needs and educate the patient on their options. The Patient Navigator will assist in removing any barriers associated with access to care. The Patient Navigator will manage follow-up appointments to coordinate continuation of care and linkages to community resources. The Patient Navigator will also work alongside the Recovery Specialist team to support the patient and ensure the highest quality of care. Qualifications: Required:
