Hospital Care at Home is an innovative care model that allows us to provide inpatient level care to patients in their homes. Potential patients are assessed by a Home Hospital provider to ensure their diagnosis can be treated in the home, that the patients home environment is a safe place to receive care, and that the patient has consented to participate. We have successfully cared for more than 500 patients in Hospital Care at Home and are excited to continue to grow!

The Home Hospital RN will partner closely with the attending physician and other members of the care team to provide care for patients, both virtually and through in-home visits. Primary responsibilities include:

  • Participate in the initial and ongoing assessment of patients determining care and services needed to stay in the home.
  • Collaborate with medical providers, patient care staff and skilled services in the planning and implementation of patient care plan.
  • Direct and coordinate services (i.e., nursing, therapy, care management, etc.) provided in the home during the inpatient admissions.
  • Collaborate with the clinical team for initiation of remote monitoring services. Monitor and manage alerts, provide education and assess patient understanding for use.
  • Act as a central point of contact for all patients, including interaction with medical, nursing, and ancillary services
  • Develop and coordinate individual nursing plan-of-care including patient/family education and discharge planning.
  • Drive to patients homes within Franklin and the six contiguous counties in a university-provided vehicle to provide in-home care.
  • May support provider with patient identification and enrollment activities.
  • Participate in ongoing improvement initiatives to foster a culture of high quality and patient safety

Minimum Qualifications

For Hire:

  • Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program required.
  • Current Ohio RN license or interim permit.
  • Minimum of 3 years clinical experience in the acute care setting, preferably in a medical-surgical, progressive care environment or emergency department. *Experience in relevant care setting preferred.
  • Valid United States or Canadian drivers license.
  • Ability to work independently, be self-directed and adapt to unpredictable circumstances.
  • Effective interpersonal, verbal and written communication skills.
  • Ability to problem-solve and iterate process improvements in delivery of home-based hospital level care
  • Computer savvy and effectively utilizes technology, including Remote Patient Monitoring (RPM) to gather, report and analyze clinical data.

On-going:

Maintains current Ohio RN license and valid United States or Canadian drivers license. Meets mandatory educational and health requirements. Demonstrates competence in technical, interpersonal, and cognitive skills required to meet essential job functions.

Supervisory Responsibility: N/A

Contacts: Interacts with physician leaders, attending and resident staff, medical students, inpatient nursing, case management and social work staff, Medical Information Management staff, financial services and other members of the health care team, patients and families.

Responsible to: Home amp; Post-Acute Operations Consultant, Hospital Care at Home Nursing Lead

  • Transport/ move patients to provide care,
  • Move objects weighing up to 40 pounds around the work environment to provide care (does not include patient movement or transfers)

  • Set up/ adjust various patient equipment items, and
  • Communicate clearly to facilitate the patients plan of care.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to use hand to finger, handle or fell objects, tools or controls; and reach with hands or arms.

Specific vision abilities required by the job include close vision, distance vision, depth perception, and the ability to adjust focus.

Work Environment

  • The work environment involves risks or discomforts which require special safety precautions, additional safety education and/or health risk monitoring, e.g. chemicals, radiation, working with sharps (needles, syringes, scalpels), infectious diseases. Employees may be required to use protective clothing or gear such as masks, gowns, goggles, gloves, etc.
  • The work environment involves everyday risks or discomforts which requires normal safety precautions typical of such places as offices and meeting rooms (i.e. use of safe work practices, avoidance of trips and falls, and observance of fire regulations and traffic signs).
  • The work environment involves risks associated with the use of computers such as eye strain and disorders that result from repetitive motions associated with computer use.
  • The work environment involves risks associated with operating motor vehicles such as vehicular accidents and hazardous road conditions (eg. Weather-related, night-driving, etc.).
