Summary
At Windemere Nursing & Rehabilitation Center, our CNAs play a vital role in providing compassionate, high-quality care to our residents. The primary purpose of this position is to deliver routine daily nursing care and support in accordance with each resident's care plan and Windemere's established procedures and standards.
This position requires direct patient care and offers opportunities to make a meaningful difference in the lives of our residents every day.
Essential Functions
Provide daily personal care and assistance to assigned residents following individualized care plans.
Accurately document all care activities and observations on required flow sheets, notes, and charts.
Promptly report any changes in residents' conditions, accidents, or incidents to the nurse on duty.
Maintain residents' comfort, dignity, and privacy in all care interactions.
Assist with activities of daily living, including bathing, dressing, grooming, toileting, and mobility support.
Measure and record vital signs, intake, and output; report significant findings to nursing staff.
Prepare residents and rooms for admissions, transfers, discharges, and meals.
Support residents with feeding, hydration, and nutritional needs; document intake and report changes.
Assist with positioning, turning, lifting, and transferring residents safely, using mechanical lifts when required.
Observe and report any signs of skin breakdown or pressure areas; assist with preventative care routines.
Provide catheter and perineal care, as well as range-of-motion exercises, as directed.
Participate in shift-to-shift walking rounds and communicate effectively with team members.
Maintain infection-control practices, including Universal Precautions and hand hygiene.
Attend all required in-service education, orientation, and training programs.
Uphold facility policies on safety, confidentiality, and resident rights, ensuring compliance with HIPAA and OBRA regulations.
Contribute to a positive and team-focused environment, supporting peers and promoting resident well-being.

Education
High School Diploma or Equivalent required

Licenses and Credentials
Certified Nursing Assistant/Patient Care Technician [CNA/PCT] - Data Conversion - Various Issuers required

Experience
Geriatric and/or Long Term Care experience preferred but not required of serving the same age/type of resident population served by this facility. New graduate CNA's will be considered on an individual basis. 0-1 year required

Knowledge, Skills and Abilities
- Must possess a current, unencumbered CNA certification to practice in Massachusetts.
- Must be able to read, write legibly, speak clearly and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action in a timely fashion.
- Must possess the ability to deal tactfully with staff, patients, family members, visitors, government agencies/personnel and the general public.
- Must possess leadership ability and the willingness to work harmoniously with professional and non-professional personnel and the general public.
- Must possess the ability to plan, organize, develop, implement, and interpret the program goals, objectives, policies, procedures, etc. of the QI/IC department.
- Must maintain the care and use of supplies, equipment, etc., and maintain the appearance of the department.
- Must have patience, tact, cheerful disposition and enthusiasm.
- Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
