Clinical Patient Services Coordinator/Scheduler LPN

Job Description:

Patient Care Coordination: Clinical Coordinator LPNs are responsible for coordinating patient care activities, which include scheduling appointments, managing patient records, and ensuring that healthcare practices comply with established standards. Patient care as needed.Communication Liaison: They act as a bridge between patients, families, and healthcare providers, facilitating communication to ensure comprehensive care delivery. This includes addressing patient inquiries and concerns. Administrative Support: In addition to clinical duties, Clinical Coordinator LPNs assist with administrative tasks, such as maintaining accurate patient records, managing documentation, and ensuring compliance with healthcare regulations. Quality Improvement: Clinical Coordinator LPNs may also participate in quality improvement initiatives, helping to enhance patient care and clinical operations within their facility.

Duties:

  • Maintain inpatient charts in the approved chart form sequence while the chart is current.
  • Medical records are updated daily as new information becomes available. Updates are filed in the correctly designated area of the chart.
  • Promptly schedules both on-campus and off-campus appointments/diagnostic tests.
  • Keeps a written log of patient appointments.
  • Communicates with admissions for each off-campus appointment.
  • Communicates discharges and cancelled appointments to admissions and receptionist.
  • Assumes responsibility to obtain written prescription and referrals as they are required.
  • Answers the telephone promptly, identifying self and location.
  • Receives information and takes telephone messages in a pleasant, professional manner and channels important telephone calls directly to the appropriate individuals.
  • Is at all times aware of the need for patient confidentiality.
  • Keeps stationary supplies available to the department as needed.
  • Oversees and participates in the daily housekeeping of the nursing station.
  • Assists in department coverage, as assigned. (Treatment Plan Updates)
  • Provides clerical support: including labeling files and charts, filing forms in patient chart and any general clerical duties.
  • Maintains the daily department census sheet/board.
  • Maintains in order and safely stores those department forms and documents that are not part of the medical record
  • All other duties as assigned

Qualifications

Requirements:

GA LICENSE

Minimum 2 years experience as an LPN. Experience in an office setting and behavioral health preferred.
