Job Summary

Under minimal supervision, performs outpatient clinic nutrition assessment, re-assessment, and education for patients in the Bariatric Surgery program at Denver health. This position serves in a multi-disciplinary team to meet the program requirements for patients to obtain bariatric surgery. This is a PRN position, up to 32 hours per week.

Essential Functions:

  • Assist patients with menu planning and meeting appropriate caloric needs (5%)
  • Performs individual and group nutrition education according to established standards of care (5%)
  • Proficiently performs assessments for "at risk" patients and completes within 72 hours of assessed need, referral, risk notification, or MD consult. (5%)
  • Proficiently performs re-assessments for "at risk" patients and completes according to level of nutrition risk time frames. (5%)
  • Inputs orders for nutrition support as indicated (5%)
  • Prioritizes workload appropriately. (5%)
  • Provides education with respect to the patient/family's education level, optimal learning tool(s)/techniques(s) and readiness to learn. (5%)
  • Provides appropriate education and documentation within 48 hours of assessed need. (5%)
  • Provides patient education according to policy. (5%)
  • Participates in hospital activities to maintain high quality patient care and services. (5%)
  • Regularly participates in physician rounds, discharge planning and team conferences as a multidisciplinary team member to ensure optimal patient care. (5%)
  • Participates in quality improvement and/or patient care committee/task force as assigned by supervisor. (5%)
  • Completes monthly productivity logs. (5%)
  • Performs PI/CQI projects as assigned by supervisor. (5%)
  • Conducts monthly tray evaluations and patient visitations according to schedule. (5%)
  • Routinely identifies areas makes suggestions to improve service and care. (5%)
  • Communicates effectively with patients, visitors, peers, medical staff and other employees. (5%)
  • Effectively obtains patient information and handles patient/customer requests and complaints (5%)
  • Documents patient care activities on appropriate forms and according to accepted hospital practice. (5%)
  • Effectively uses computer software/programs/electronic medical record to perform diet office functions and clinical activities. (5%)
  • Performs intake analysis (calorie counts) as needed. (0%)
  • Accurately clinical activity data into computer tracking program. (0%)
  • Assists with monitoring the diet techs and diet office personnel by providing guidance and assistance to personnel with problem resolution. (0%)
  • Maintains ongoing professional education through participation in meetings, seminars, in-services and literature review to enhance nutrition knowledge, and professional development and practice. (0%)
  • Attends a minimum of 1 professional meeting and/or seminar per year for each clinical specialty and/or age-specific population of the clinician's area of service. (0%)
  • Serves as a resource, provides education, assistance and/or training/in-services to departmental staff on information obtained from educational meetings/in-services. (0%)
  • Serves as a resource, provides education, assistance, and/or training/in-services to hospital as assigned. (0%)
  • Has no incidents where employee is asked to redo work because of poor performance. (0%)
  • Performs at appropriate level of productivity, adhering to break/lunch times as assigned. (0%)
  • Functions as a team player in the department knowing when to ask for assistance as needed and being able to assist when required. (0%)

Education:

  • Bachelor's Degree required

Work Experience:

  • 1-3 years related experience in clinical or public health nutrition required

Licenses:

  • Registered Dietitian Certification , Commission on Dietetic Registration required

Knowledge, Skills and Abilities:

  • Effective communication skills in both oral and written form.
  • Demonstrates ability to delegate responsibility as appropriate.
  • Ability to exercise good independent judgment.
  • Ability to follow written and oral instructions. Process the ability to assess data reflecting the patient's status and interpreting appropriate information.
  • Demonstrates knowledge of disease specific and age specific nutrition standards of care.
  • Demonstrates understanding of Local, State, Federal and The Joint Commission standards and regulations.
  • Demonstrates competency with computer system for order entry and to update diet and patient information.
  • Demonstrates understanding of how to use most current version of on-line Nutrition Care Manual.
  • Ability to use Microsoft Word, Outlook, Excel and other IT/software systems.
