A Brief Overview The Social Work Department is seeking a part-time Master's prepared social worker for the Home Care/ Home Health Unit at Children's Hospital of Philadelphia located in Suburban Philadelphia. This social worker will provide services to pediatric patients and their families receiving care from the Home Care Department. These social work services include assessment of psychosocial functioning, supportive counseling and education, referral to community resources, and help with accessing benefits, collaboration with medical professionals, and support with education resources. Social Work experience in medical social work, home care or medical case management is preferred. Some travel will be required for this position. Part time position - 24 hours/week
What you will do
- Provides skilled psychosocial assessment and collaborates with care team in order to provide quality comprehensive treatment to patients and families. Evaluates the effectiveness of professional services and adjusts/redefines goals and modalities as indicated. Shares information regarding patient family situation with staff from other agencies and programs as appropriate in accordance with applicable confidentiality guidelines and relevant law. Organizes workload and sets priorities based on patient care needs. Adheres to medical-legal reporting requirements.
- Completes documentation in a timely manner that is legible, clear and organized according to department, hospital and legal standards.
- Provides leadership in identifying barriers to effective service delivery or problems in work assignments. Actively contributes in initiatives that improve service delivery through department, hospital or community based committees.
- Recognizes professional strengths and areas for growth and is accountable for development and enhancement of professional practice skills.
- Adheres to NASW Code of Ethics and CHOP Social Work Department standards for continuing education, participation in professional home committees, meetings and available for cross-coverage assignments as needed.
Education Qualifications
- Master's Degree Social Work - Required
Experience Qualifications
- Previous experience in a hospital, health organization or mental health agency setting - Preferred
Licenses and Certifications
- Licensed Social Worker (LSW) (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Preferred
- Membership in the Academy of Certified Social Workers (ACSW) - Academy of Certified Social Workers (ACSW) - upon hire - Preferred

$67,500.00 - $86,060.00 Annually
