Position Overview: The Admissions Coordinator plays a vital role in managing the admission process for patients entering the skilled nursing facility. This position involves coordinating referrals, verifying insurance, and ensuring a seamless transition for new residents. Key Responsibilities: * Review clinical documentation to determine level of care needed. * Verifies physician orders. * Obtain pre-admission documentation from referral sources to determine if agency can care for the patient adequately. * Communicate with patients, families, and healthcare providers to gather necessary documentation. * Maintain accurate records in electronic medical systems. * Coordinate scheduling and follow-up for new admissions. Qualifications: * Previous experience in healthcare admissions, intake, or patient services preferred. * Strong organizational and communication skills. * Ability to multitask in a fast-paced environment. * Proficiency with EMR systems and Microsoft Office Suite.
