Responsibilities

Foster Family Coordinator - The ideal candidate must reside in the West TN area of the state.

Additional counties to include, but not limited to, Crockett, Gibson, Madison, Hardeman, Tipton, Haywood, Lauderdale, Fayette and Shelby.

Universal Health Services, Inc. company, in partnership with Greystone Foster Care, has a long history in providing comprehensive behavioral health care services to at-risk youth struggling with extensive mental health issues. Greystone Foster Care recruits foster parents and offers the training to be equipped to provide a safe, stable and therapeutic environment for children and youth that have been removed from their homes. Our training prepares staff and foster parents to support children and youth as they begin the healing process. Our therapeutic foster parents link children and youth to hope, healing and trust.

Greystone Foster Care is seeking a dedicated, compassionate Foster Family Coordinator to add to our growing team. This is a hybrid job blends remote work and in-office position with occasional requirements to report to the facility. This role requires full-time dedication and must be flexible to include weekends and evening work.

Job Duties/Responsibilities:

  • Conducts monthly visits to each assigned foster home/parent; and collect monthly paperwork per policy as required
  • Conducts quarterly reviews on fosrer parents/homes tht include safety checks, and updating various documents, to ensure home is complying per policy
  • Assist in the corrdination of the annual training required for the approved resource parents, work with the Department of Children's Service (DCS) training staff and maintain an excel spreadsheet for complete training courses
  • Works on all aspects of Home Study as required by the Tennessee Department of Children's Services for new foster parents
  • Maintain the resource home file, keeping all pertinent documents current and prepared for review by DCS during audits or onsite visits
  • Develops a Performance Improvement Plan as needed, monitors implementation of the plan, and locates all services needed to ensure that the plan goals are met
  • During visits to assigned home, completes appropriate documentation, and checks home for any concerns per policy
  • Completes addendums and other appropriate paperwork if there are any changes within the home; children turning age 18, divorce or separation, military deployment or family members that move in or out of the home
  • Monitors the Special Investigation (SIU) status on approved homes; attends the monthly meeting with the Foster Home Quality Team (FHQT) regarding SIUs with DCS; ensure that all recommendations made by the FHQT are followed to ensure the home becomes compliant and the SIU is completed. Maintain an excel spreadsheet of findings
  • Participates in on-call schedule to provide on-call services to foster parents. Reports and documents on any on-call activities for the next business day. Calls Foster Care Director after hours for consultation as needed
  • Handles all adoption requirements as part of the permanency plan including drafting all paperwork along with initiating recruitment efforts, if needed, as part of the adoption process. Works with attorney to set court date, delivering court packet to judge in timely manner for review, along with attending the adoption hearing
  • Provide support to the Case Manager and/or Therapist working within the home and address any concerns they may have with the resource parent regarding the home
  • Performs other duties as assigned/required by this position

Qualifications

Requirements:

  • Bachelor's degree in Social Work or related field; Master's degree preferred
  • One to three years' related experience and/or training, or equivalent combination of education and experience; three to five years preferred
  • Valid state Driver's License
  • Automobile insurance with verification of medical liability.
