The Patient Resource Coordinator assists in creating positive outcomes for the patient and family by helping them navigate through the challenges faced with their health care journey. The Patient Resource Coordinator supports the Social Worker with assessments, assists the clinical care team and Social Worker to design supportive care plans, and provides education and consultation regarding community resources. Provides patients and family members within the service line information to adhere to the health care treatment plan and promote overall wellbeing. Arranges internal support groups and related functions. Serves as a liaison between the health care system and the community. EDUCATION/EXPERIENCE REQUIRED: Associate's degree or two years college/university credits. Bachelor's degree preferred. At least three years of work experience. Health care experience preferred.
