Position Summary
This position is responsible for delivering specific palliative and pastoral care to patients, their families, students, and staff. This position is responsible for ensuring that the religious and spiritual needs of patients, their families, students, and staff are met. Collaborate with clinical units regarding palliative and other pastoral care issues including chronic illness, end of life issues, death ministry, advance directives, sacramental and other pastoral requests that are made; collaborate with Nursing Administration regarding automation of nursing documentation and assessment; collaborate with palliative care team; collaborate with pastoral Care Department regarding pastoral requests; collaborate with community religious institutions and staff regarding formal worship services and religion specific pastoral care.

Duties & Responsibilities
  • Provide palliative pastoral ministry (in a timely manner) to patients/families and clinical personnel taking into consideration multiple elements of cultural and ethnic diversity and social conditions, without imposing one's own perspectives.
  • Assess patient and family spiritual needs and assist families in using their faith and/or spiritual resources in dealing with illness, trauma, and stress; visit newly admitted patients and patients with special requests; visit critical care areas; follow up with patients from previous visits as appropriate. Provide crisis intervention as necessary.
  • Provide a range of skills, including empathic reflection, listening/attending, and appropriate use of religious/spiritual resources. Document consultation outcomes and retain records per Spiritual Care process.
  • Maintain a working relationship between Spiritual Care and medical staff, nursing staff, and other departments.
  • Contribute to the department's plan for continuous quality improvement (CQI).
  • Support an interdisciplinary approach to Spiritual Care.
  • Participate in monthly staff meetings. Participate in regular professional development activities. Participate in appropriate committees. Empathy for the problems experienced by patients and their families; a commitment to help those in distress. The interpersonal skills needed to work with distressed and anxious patients, families, and staff.
  • Demonstrated computer literacy; ability to document & retrieve information through computer applications.
  • Ability to work independently and exercise good judgment.
  • Possess excellent interpersonal skills in order to establish and maintain professional relationships with members of the health care team.
  • Perform other related duties and participate in special projects as assigned.


Qualifications:

Minimum Qualifications
  • Master's degree from an accredited theological school or religious studies, or equivalent, ordination or commissioning for ministry by a recognized religious group, and a current endorsement for chaplaincy by a recognized religious group required.
  • Minimum of one (1) year clinical chaplaincy experience required.
  • Must be a Board-Certified Chaplain. (Palliative Care Certification must be obtained within 36 months of hire.)
  • Awareness of self as pastoral caregiver, including attitudes, values, assumptions, strengths and weaknesses, and the ways that ministry affects people.
  • Knowledge of the hospital/clinic system and multi-disciplinary teams.
  • Empathy for the problems experienced by patients and families; commitment to help the distressed. The interpersonal skills needed to work with distressed and anxious patients, families, and staff.
  • Demonstrated computer literacy; ability to document & retrieve information through computer applications. Ability to work independently, and exercise good judgment.
  • Possess excellent interpersonal skills in order to establish and maintain professional relationships with members of the health care team.


Preferred Qualifications
  • Preferred experience with advance directives (POAHC, Living Will, POLST)
  • Preferred formal education with ethics
  • Preferred Palliative Care Certification
