Job Summary
The Social Worker provides services to our patients and / or their families in order to meet their
medically related social and emotional needs. The Social Worker recognizes the importance of
meeting these needs for the patient and their families to promote health and reduce the
impingement of these needs on the patient's medical condition, treatment, and recovery. The
Social Worker coordinates with other healthcare professionals to meet the patient's discharge
needs to support the patient and their family with the transition from one healthcare setting to
another. The Social Worker provides social work intervention with the intent to improve or resolve
patients' psychosocial problems related to their medical care. The Social Worker participates in staff
and community education and provides employee assistance services. The Social Worker supports
an environment of caring focused on healing and wellness.

Minimum requirements

Master's in social work and one year of experience
