As a Kidney Transplant Dietitian, you will be responsible for planning, organizing, and evaluating all phases of nutritional care for pre-transplant as well as post-transplant patient populations at Carilion Roanoke Memorial Hospital. You will collaborate with the transplant team to help coordinate and maintain a high standard of nutritional care and provide advice regarding patient candidacy. What you will do in this role: As a Kidney Transplant Dietitian you will perform nutritional assessments, develop plans of care and educate patients and caregivers as appropriate. You will participate in interdisciplinary meetings with the transplant team members to deem candidacy. You will be responsible for assessing patient needs considering the disease process as well as dialysis needs for patients undergoing transplant evaluation and follow up. You will determine recommendations for nutritional status improvements and provide direction and instruction for resolution of nutritional barriers to transplantation. You will maintain documentation in the patient's medical record of all interventions and recommendations. You will collaborate with the medical and interdisciplinary team to optimize nutritional management along the continuum of care, including prior to surgery, during the patient's inpatient hospitalization, and in the recovery and maintenance phase. Job description summary may include: Plans and provides medical nutrition therapy and client education for inpatients and outpatients ranging from neonates to geriatrics. Conducts individualized nutrition assessment and evaluation and implements appropriate medical nutrition therapy approaches. Provides advanced and specialized nutrition support and communicates recommendations for changes in patient treatment plan to physicians and hospital staff. Documents recommendations and monitors changes and patient outcomes. Facilitates the development and implementation of policies, procedures, and protocols related to nutrition care. Evaluates and tracks data for outcomes and related performance improvement measures and makes recommendations to improve quality. Develops and oversees patient and staff educational materials regarding evidence-based medical nutrition therapy. Serves as the nutrition resource to the healthcare team. Additionally, as an experienced practitioner, has an area of responsibility which supports the facility, organization, community or nutritional team to improve the health of our patients and community. Acts as a resource to other staff and presents educational programs.
