Job Summary The Comprehensive Community Services (CCS) Rehabilitation Specialist is a member of the CCS team who provides individualized skill development and enhancement services to support people in achieving their recovery goals. This role serves individuals at various stages of recovery from mental health, substance use, and co-occurring needs, using a strength-based, person-centered approach. Role varies by team, and services are delivered in community settings based on goals identified in the Individual Recovery Plan. Essential Duties and Responsibilities
- Serve as a member of the CCS team, providing person-centered services to individuals with mental health, substance use, and co-occurring needs, based on their strengths and recovery goals.
- Deliver individual skill development services such as communication, problem solving, decision making, self-regulation, and conflict resolution, as outlined in the Individual Recovery Plan.
- Provide training in daily living skills including personal care, housing, benefits, household tasks, financial management, transportation, parenting, and accessing community resources.
- Use varied methods such as role modeling, mentoring, monitoring, assistance, and cuing to support individuals.
- Offer services in community, including individuals' homes and other relevant locations, and ensure transportation for CCS-related appointments.
- Collaborate in Recovery Team meetings to help develop and monitor individualized recovery plans and goals.
- Maintain accurate and timely documentation in compliance with agency policies, government regulations, and applicable local, state, and federal codes.
- Attend staff meetings, training, and other required events, and maintain flexibility in schedule to meet program needs while following all agency policies and procedures.

Required Qualifications
- Bachelor's degree in social work, psychology, or a human service-related field is required.
- 2 years of experience serving people with mental health, substance use, and/or co-occurring needs preferred.
- Meet all WCS and contractor requirements for criminal and caregiver background checks, driver's record, and drug screen.
- Valid driver's license, automobile, and sufficient insurance to meet agency requirements.
Knowledge, Skills, and Abilities
- Adhere to the agency Code of Conduct, all relevant policies, procedures, and funding requirements. Act ethically and maintain professional, positive relationships with staff, individuals served, and external partners.
- Demonstrate cultural sensitivity and a strong commitment to the agency's mission and values.
- Possess knowledge and experience working with individuals in recovery with mental health, substance use, and co-occurring needs.
- Display strong organizational, assessment, and interviewing skills; build rapport quickly and navigate difficult situations effectively.
- Remain flexible, able to multitask, and use time and resources efficiently to support high-quality CCS service delivery.

PHYSICAL DEMANDS: Position is mobile with significant time spent in the community, including transporting individuals being served, home visits and attending meetings; driving throughout Milwaukee County. Ability to navigate stairs. WORK ENVIRONMENT: Work is performed primarily in the community, including people's homes and various community-based locations. Ability to work in an office setting.
