As our hospice Subject Matter Expert, you'll be the goto professional when patients and care teams need guidance, comfort, and clarity. You'll support patients who are too ill to return home, ensuring they receive the dignity, comfort, and love they deserve at the end of life. You'll make a profound impact, walking beside patients and families during their most meaningful moments and ensuring every transition is handled with exceptional care. Position Type: Full Time Salary: $95000 - $110000 / year Schedule: Sunday - Thursday 10am - 7pm or Tuesday - Saturday 10am - 7pm Offer Based on Years of Experience What You Need to Know Key Reponsibilities: Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily inperson collaboration and communication. Serve as the primary clinical resource for hospice education, eligibility, and appropriateness. Conduct patient assessments and coordinate the admission process. Partner with Admissions, Clinical, and Care Teams for smooth transitions. Advocate for patients and families during goalsofcare conversations. Participate in interdisciplinary planning, training, and outreach. Uphold ethics, quality, and continuous learning. Meet expectations for response times, referral coordination, and communication. Be the Best Inpatient Hospice Clinical Liaison Registered Nurse RN You Can Be If you meet these qualifications, we want to meet you! One year of RN experience with knowledge of home health regulations, Medicare requirements, and careplan development Medical sales or clinical outreach experience preferred Strong communication and relationshipbuilding skills Ability to work independently in a hospital environment Passion for serving with dignity and empathy Active RN license Valid driver's license and insured vehicle Our Investment in You Caring for others starts with caring for you.
