About Duke HomeCare & Hospice Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC. External candidates eligible for a $5000.00 Commitment Bonus paid over 1 year JOBSUMMARY Provides psychosocial assessments, intervention, and /or treatment related to the patient's illness, and/or life situation. Identifies community resources to assist and meet the continuing needs of the patient and family in the home environment. May evaluate patients referred to the hospice program for admission MAJOR JOB RESPONSIBILITIES
PREFERRED QUALIFICATIONS Education and License Work requires a minimum of a Bachelors in Social Work from CSWE accredited school of social work. Masters of social work from an accredited school of social work, preferred. Work Experience 2years of social work experience, in a health care setting. Computer skills required with Microsoft Word and Excel experience preferred. Knowledge, Skills, and Abilities
