Come talk to us about joining the Centerstone team! JOB DESCRIPTION: SUMMARY OF POSITION The Steven A. Cohen Military Family Clinic offers a variety of services to post-9/11 Veterans, Active-Duty Service Members, Reservists, National Guard and their family members. The master's Level Case Manager is part of a multidisciplinary clinical team who serves to bridge between clinical services and essential community resources as well as provides individual, couples, family and group therapy to our military families. The Case Manager will provide case management services in-person and in a virtual platform. The Case Manager facilitates direct referrals for housing, employment, healthcare, legal, educational, peer-support and all other essential community-based needs for clinic clients. The Case Manager manages referral partnerships by continuous engagement post-referral, ensuring the provision of high-quality care. The Case Manager assists the clinical team in understanding resources and needs within the clinic community. The master's Level Case Manager will additionally provide clinical treatment to the military population served via a caseload of clinical cases. Additionally, the Case Manager will provide an array of clinical treatment to post-9/11 Veterans, Active-Duty Service Members, Reservists, National Guard and their family members. The Case Manager will provide evidence-based therapy to individuals, couples, families and groups.The Steven A. Cohen Military Family Clinic has a commitment to training, consultation and proficiency in Cognitive Behavioral Therapy, Cognitive Processing Therapy, and Prolonged Exposure, through which the Case Manager will deliver short-term treatment, targeted treatment. The Case Manager will support the Centerstone Mission, Vision and Values in delivering care that changes people's lives and is expected to communicate clearly and appropriately, providing excellent customer service. Adherence to applicable policies and procedures is expected. Regular attendance and punctuality are required. This position requires Wednesday evenings until 7pm. ESSENTIAL DUTIES & RESPONSIBILITIES
