Shift Schedule: Night shift 11:00pm-7:30am- Day shift 7:00am-3:30pm -Evening shift 3:00pm-11:30pm

Job Description:
  • Provided nursing care under the supervision of a registered nurse or physician, delivering treatments in accordance with individualized care plans to maintain health and support recovery.
  • Administered medications, feedings, dressings, and other nursing procedures; reviewed MARs, counted medications, and reported discrepancies.
  • Monitored and documented vital signs, symptoms, health status changes, and responses to treatment; promptly reported updates to the RN/physician.
  • Assisted in developing and implementing care plans by identifying individual needs and participating in ISP meetings as needed.
  • Supported individuals with activities of daily living (ADLs) including bathing, dressing, grooming, feeding, and hygiene, while promoting independence and dignity.
  • Prepared individuals for medical examinations and treatments; provided education and counseling to individuals and families regarding health, medications, and care plans.
  • Maintained accurate and timely documentation of all care, including charts (weight, bowel, menses, medication sheets, and medical records) in compliance with policies.
  • Conducted regular health assessments, including weekly visits and follow-ups, ensuring continuity of care and addressing changing needs.
  • Communicated and coordinated with healthcare teams, including RNs, physicians, and staff, serving as a liaison to ensure effective care delivery.
  • Supervised and supported direct care staff to ensure compliance with care standards and proper medication administration .
  • Assisted in maintaining a safe and clean environment, including light housekeeping and supply monitoring.
  • Utilizes and keeps current medical tracking forms.
  • Works with designated house staff to ensure that medical appointments are scheduled as indicated and adhered to in a timely manner.
  • Provides a completed, comprehensive consultation report for the staff to take on the medical appointment.
  • Reviews the consult recommendations and discusses with the primary MD as indicated in a timely manner.
Required Skills:
  • Reviews MD orders monthly and verifies accuracy on MAR. Compares new MAR against previous month to ensure changes are carried over. Initials the MD orders and MAR denoting accuracy.
  • Weekly, reviews the MARs for accuracy and completeness. Initials MAR at the end of the month and files into consumer chart after verifying that it is accurate and complete. Follows up when discrepancies noted.
  • Ensures that medication is ordered by staff in a timely manner.
  • Ensures that a med inventory is conducted and maintained in accordance with nursing policy.
  • Establishes and maintains a complete, clean medication station with adequate supplies.
  • Ensures that meds are logged in and checked upon pharmacy receipt and reports any pharmaceutical dispensing errors to the Mental Hygiene Nurse Program Coordinator (NPC).
  • Provides information to the Approved Medication Administration Personnel's (AMAPs) on the medication prescribed; their expected effects and possible side effects (client specific medication sheet).
Infection Control Surveillance:
  • Notifies Residential Nurse and Infection Control Nurse of any infectious disease processes e.g. antibiotic therapy, TB conversions, exposure incidents in the residence or at day program.
  • Reviews and maintains immunization standards for individuals; documents updates on the immunization record and Medical Information Summary.
  • Adheres to & practices standards as set forth in Infection Control Guidelines
  • Completes all nursing tasks in a comprehensive and timely manner:
  • Completes assignments in an efficient manner and consistent with the time frames allotted.
  • Provides back up nursing coverage when primary nurse is off duty and contacts the home(s) to determine health care needs at least weekly.
  • Maintains a current Nursing Policy and Procedure Manual; ensures its accessibility to staff within assigned homes.
  • Maintains certification in CPR.
  • Maintains current LPN license.
  • Maintains certification in Infection Control for Health Care Workers.
  • Medications and tube feeding task as explained in Exhibit B.
  • LPN has an accepted and recognized role in the coordination of care required by patients with chronic, catastrophic or complex high-risk or high-cost health care issues.
