The Nurse Health Advisor supports the primary care team to deliver preventive care for patients. Working under close supervision, this role supports the development of behavioral changes and care plans in order to improve health outcomes and close gaps in care in accordance with Value Based Care. Essential Job Function Assists with identifying eligible patients for Medicare Annual Wellness Visits and Chronic Care Management (CCM) and documenting care information in the Electronic Medical Record (EMR). Educates patients on health improvement through behavior changes by working to develop care plans and treatment goals. Collaborates with the Care Guide to identify and schedule patients for screenings, tests, and immunizations. Supports quality improvement by reviewing data, including evaluations of interventions, in order to improve the effectiveness of care. Communicates with patients who have upcoming appointments in order to establish pre-visit planning and understand any gaps in care. Assists in the identification of Triad HealthCare Network (THN) and other high-risk populations who have been admitted to the hospital in order to communicate with them regarding questions and follow-up visits. Performs other duties as assigned. Education Required: High School Diploma or equivalent, Graduate of a Licensed Practical Nursing program. Experience Required: 3 years of related experience. Licensure/Certification/Listing Required: LPNBLS (CPR)-American Red Cross or AHA Healthcare Provider.
