POSITION SUMMARY:

The Care Management Department is offering experienced nurses an exciting opportunity to transition into the specialized field of care management. As a dynamic and diverse team, we provide comprehensive care to patients across the BMC continuum. We seek dedicated nurses who are passionate about serving our unique patient population and motivated by opportunities for professional growth. Ideal candidates have experience in quality improvement initiatives or active participation in unit-based or hospital-wide councils. We also value multilingual proficiency and a commitment to lifelong learning, including engagement in conferences, volunteer work, or other professional development activities.

Our Nurse Residency Transition Program is designed to equip experienced nurses with the knowledge, skills, and confidence to excel in care management. This structured program includes focused classroom instruction and a 12-16 week precepted clinical experience tailored to support your transition. Both the didactic and hands-on components emphasize the competencies needed to provide comprehensive discharge planning and case management for complex patients. Upon completion, participants will be well-prepared to advance as highly skilled Care Managers.
Participants in the transition program will receive their regular hourly pay throughout the training.


  • This schedule provides maximum exposure to the new clinical environment, colleagues, and patient populations.

  • Each nurse is responsible in collaboration with their preceptor and clinical nurse educator to devise weekly goals.

  • Basic computer proficiency is inclusive of the ability to access, enter, and interpret computerized data/information.

  • Effective interpersonal skills to facilitate communication with various healthcare team members, patients, and families.

  • Organizational skills to set priorities and efficiently complete assigned work.

  • Relevant clinical experience and/or specialty, certification is preferred for certain practice areas.

  • Analytical ability to solve clinical nursing issues and conduct quality improvement projects.


DEPARTMENT: Care Management

REGULAR SCHEDULE: Days, 10 hour shifts, every sixth weekend

ORIENTATION SCHEDULE: 40 Hours, TBD (must be available to work any shift for 12 weeks, including weekends, until the completion of training)

APPLICATION TIMELINE AND REQUIREMENTS:


  • 12-16 weeks competency based orientation, specific classes and precepted shifts

  • Nursing degree: Diploma, ASN or BSN (preferred), Ability to obtain BSN within 4 years

  • Must have active or pending Massachusetts Registered Nurse (RN) license

  • 2+ years of RN experience required


  • Experienced RNs who wish to apply for the posted positions will be required to submit:



    • A current resume;

    • A letter of intent;

    • One Professional Reference Questionnaire from either a current or recent BMC manager, Clinical Educator or BMC colleague who can speak to the applicant's knowledge, ability, collegiality and commitment to excellence in patient care. The Professional Reference Questionnaire can be found on the BMC Nursing homepage.





  • Nurses with experience in quality improvement initiatives or active participation in unit-based or hospital-wide councils are highly preferred. Additionally, we prioritize nurses with multilingual proficiency and those who actively pursue knowledge in case management through conferences, volunteer work, or other professional development opportunities.

  • The application packages will be reviewed and selected candidates will participate in a non-compensated training shadow experience. Such experience shall include shadowing another RN in the Care Management Department for a period of at least 4 hours to gain firsthand insight into the role.


Compensation Range:

$38.05- $75.33

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, and licensure/certifications directly related to position requirements.
