Acute Care Pharmacy Job Summary

The year 2 (PGY2) Pharmacy Practice Residency program is a 12-month post-graduate training program which builds on the Doctor of Pharmacy (PharmD) education and post-graduate year 1 (PGY1) training. The PGY2 residency will contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions making them eligible for board certification.
Under the general supervision of the PGY2 Residency Program Director or his/her designee, PGY2 residents will have the opportunity to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care that improves medication therapy. PGY2 residents must be pharmacists having sufficiently broad knowledge, skills, attitudes, and abilities in pharmacy practice necessary for further professional development at an advanced level of pharmacy practice.
PGY2 residents will complete learning experiences in 4-to-8-week rotations in differing clinical specialty areas as well as longitudinal learning opportunities. The resident will learn from his/her preceptor and assume responsibility and accountability in providing day-to-day patient care and pharmacy medication management in a focused area of practice. Clinical activities include the development and implementation of a therapeutic care plan, follow-up evaluation and medication monitoring and proper documentation of medication related interventions.
A major and minor project will be expected to be completed over the year which will improve the overall status of pharmaceutical care of our patients at Denver Health. The resident will assume an integral role in supporting the educational and academic vision of the pharmacy department as a preceptor for students, interns and PGY1 residents. The resident will provide educational programs to pharmacy, nursing, medical and allied health care professionals regarding appropriate medication use, medication management standards, and safety in a focused area of practice.
Residents are taught and evaluated by the educational goals and objectives set forth by the American Society of Health-System Pharmacist's Accreditation Board.

Essential Functions:
  • Establishes a presence and working relationship with providers/staff in specialty practice area. Attends patient care rounds as applicable to clinical specialty area. Collaborates with providers in establishing treatment plans for patients. Assistance in the following areas: formulary or preferred agents, evidence-based medicine, drug information to both providers and patients, drug interaction screening, medication profile review, assessment of patient response to therapy. (35%)
  • Designs therapeutic drug regimens or plans based on patient-specific information that includes establishing a therapeutic endpoint, selecting appropriate drug and related therapy, and establishing an appropriate dosing regimen. Demonstrates a sound problem solving/decision making skills with practical, efficient, economical, and reasonable solutions. (20%)
  • Monitors drug therapy for potential drug-drug, drug-food, or drug-disease state interactions. Assess and monitor patient response to medication regimens. Documents all patient activities and interventions in the patient's medical record or pharmacy computer system. Demonstrates accuracy, thoroughness, and attention to detail. (15%)
  • Assists in the formulary management process at Denver Health by preparing new drug monographs, conducting therapeutic drug class or disease state reviews, development of clinical practice guidelines, or development of other policies and procedures. Participates in the education and implementation of new formulary guidelines for providers/staff in as assigned. Attends P&T Committee or other medication management committee meetings throughout the year. (10%)
  • Participates in and provides educational programs for the department or personnel as required by specific rotation preceptors. Completes an in-service presentation for pharmacy staff during each rotation. Assist in the development of competency modules for staff as assigned. Presents results of residency project at the Mountain States Residency Conference. Prepares manuscript of major project prior to completion of residency. (10%)
  • Learns with Lean tools and department quality improvement projects. Participates in a Lean event and department goal setting sessions. Reports and follow up on medication/patient safety issues in area of practice. Learns with patient safety goals or core measures that affect the pharmacy department and participate in development of programs that ensure compliance with these measures. (5%)
  • Provides education and training. Serves as a preceptor for postgraduate year one pharmacy residents, pharmacy interns, and pharmacy students on rotation at Denver Health. Attends and participates in departmental and/or system-wide meetings, in-services, huddles, and committees. (5%)

Education:
  • Bachelor's Degree BPharm and DORA CO Pharmacy license within first 3 months of residency Required
  • Bachelor's Degree PharmD and DORA CO Pharmacy license within first 3 months of residency Required

Work Experience:
  • 1-3 years Completion of a PGY1 Pharmacy Practice Residency from an ASHP accredited program Required

Licenses:
  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association
  • ACLS-Advanced Cardiac Life Support - AHA - American Heart Association
  • PALS-Pediatric Advanced Life Support - AHA - American Heart Association
  • PBID-Pharmacy Based Immunization Delivery - BPS - Board of Pharmacy Specialties

Knowledge, Skills and Abilities:

  • Excellent verbal and written communication skills, active listening skills and ability to function as a member of a highly efficient pharmacy team.
  • In depth knowledge of medication management practices and principles, pharmacotherapy, the provision of patient-centered clinical services, drug information systems, formulary management and current treatment guidelines for focused area of practice and disease states.
  • Ability to be conscientious and pay close attention to detail.
  • Good time management and organizational skills are essential for successful completion of the residency program.
  • Proficiency with Microsoft Office required.
  • Proficiency with electronic health record software, Epic preferred.
