We are seeking a Clinical Nursewho willsupport the General Internal Medicine (GIM) Faculty practice at Green Spring Station (GSS).

Specific Duties & Responsibilities

  • Coordinates care for a group of complex patients to achieve clinical and organizational outcomes.
  • Provides evidence-based care based on nursing process, policies, procedures, and protocols of the Johns Hopkins Hospital.
  • Provides guidance to other team members to promote quality, safety, and excellence in care.
  • Actively participates in Comprehensive Unit Based Safety Program (CUSP) initiatives.
  • Performs transitional care management calls for practice patients discharged from hospitals in the area and determines the need for short-term outpatient follow-up.
  • Provides direct patient care, when needed, for point of care testing, medication/vaccine administration, patient rooming and other duties as assigned.
  • Provides patient education for self-care chronic disease management, such as, diabetes education (in person, by phone, or via MyChart).
  • Works collaboratively with a multidisciplinary health care team to optimize team performance and patient outcomes.
  • Triages phone calls and electronic advice requests from patients and determines need for medical assessment.
  • Assesses lab results and implements intervention in consultation with primary providers.
  • Triages calls from outside institutions, JHM Emergency Departments, and other health care providers; determines and implements follow-up plans.
  • Identifies care continuity needs of patients; generates and pends orders to primary providers for signature.
  • Assists primary providers with maintaining electronic medical records; scans outside results into system, updates immunization history, updates health maintenance records, acts on overdue health maintenance items.
  • Conveys critical action values/ patient request of lab results when requested by provider (communication from MD to RN)
  • Assist in the development/updating of clinic-based protocols, standardized workflows, or other clinical practice procedures.
  • Helps lead quality improvement initiatives that are focused on outcomes measures that are relevant to primary care.
  • Assists Medical Office Coordinator (MOC) in scheduling patients for appointments.
  • Helps complete forms (e.g., disability, FMLA, mobility) requiring basic health history.
  • Helps obtain prior authorization for tests and medications
  • Provide oversight for ordering, stock, and maintenance of clinical supplies and equipment. Collaborates with clinic manager to ensure supplies, medications, and vaccines available and up-to-date for clinical use.
  • Responsible for other clinical duties as assigned.

Skills and Abilities

  • Expected to learn the formal and informal departmental goals, standards, policies and procedures which may include some familiarity of other departments within the school/division. Is sensitive to the interrelationship of both people and functions within the department.
  • On a regular and continuous basis, exercises administrative judgment and assumes responsibility for decisions, consequences, and results having an impact on people, costs, and/or quality of service within the functional area.
  • Exchanges non-routine information using tact and persuasion as appropriate requiring good oral and written communication skills.
  • Knowledge of nursing process; social, behavioral, biological, and physical sciences; safety and scientific inquiry process and application to the skill performance; leadership and management as normally acquired through completion of a program in nursing.
  • Knowledge of current health care environment.
  • Knowledge of general physiologic monitoring and patient care equipment such as defibrillator and glucose monitor.
  • Successfully completes all training courses required for access to Epic and other clinical software systems.
  • Requires effective use of resources and cost awareness.
  • Ability to analyze data to synthesize and summarize information and to make judgments regarding patient care.
  • Highly effective verbal communication and interpersonal skills to establish working relationships that foster optimal quality patient care.
  • Ability to use a computer keyboard to input and obtain data.
  • Ability to operate personal computer to access email, computerized scheduling and documentation systems, and other basic office support software including Microsoft Office applications.
  • Ability to apply principles of evidence-based practice, scientific process, and service excellence in practice.
  • Ability to interact successfully with the public. Ability to perform effectively despite sudden deadlines and changing priorities. Ability to maintain personal composure in high stress situations.
  • Demonstrated interpersonal, written and verbal communication skills are required to service a diverse customer base.
  • Ability to demonstrate and convey a favorable image of the organization and to conform to proper standards of professional dress, attitude and demeanor. Ability to demonstrate a high level of interpersonal skills required to interact with patients, patients' families/visitors and clinical staff.
  • Ability to perform with a high degree of accuracy and with meticulous attention to detail. Demonstrate a strong ability to use initiative and judgment and to identify, analyze and solve problems.
  • Ability to demonstrate self-direction and initiation. Ability to work independently and as a part of a team.


Minimum Qualifications
  • Individual must be a registered nurse, licensed in the State of Maryland
  • Must maintain active licensure throughout employment.
  • Experience with Epic preferred.
  • Additional education may substitute for required experience, and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula.
  • The individual must be a registered nurse, licensed in the State of Maryland or the state where practicing.
  • Current American Heart Association or American Red Cross CPR Certification.
  • Completion of the JHH Credentialing process.
