The Social Work Practitioner will serve as a core member of the UNC Bronchopulmonary Dysplasia ( BPD) Interdisciplinary Care and Outreach Program within Pediatric Pulmonology, providing psychosocial assessment, care coordination, resource navigation, and family support for infants and children with BPD and other complications of prematurity. This position will work collaboratively with pulmonology, neonatology, cardiology, nutrition, developmental specialists, nursing, and community providers to address social determinants of health and reduce barriers to care for this medically complex population. This is a 0.5 FTE position, 20 hour per week.

Key responsibilities include: assessing family psychosocial needs; providing counseling, education, and support to caregivers; facilitating access to community resources, financial assistance programs, home health services, and durable medical equipment; and supporting transitions from the NICU to outpatient care. The Social Work Practitioner will assist with screening for social determinants of health, maternal mental health concerns, transportation challenges, food insecurity, housing instability, and other factors that may impact patient outcomes.

In addition, this position will support the development of educational resources for families and community providers, participate in multidisciplinary case conferences, collaborate with regional partners across North Carolina, and contribute to program evaluation and quality improvement initiatives. The Social Work Practitioner will serve as a resource for patients, families, and healthcare teams to promote coordinated, family-centered care and improve long-term outcomes for children with BPD.

Bachelor's degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.
