Make a Difference in Advanced Heart Failure Care at Johns Hopkins!



Johns Hopkins Hospital is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our Advanced Heart Failure Program within the Heart Failure Bridge Clinic (HFBC). This is an exciting opportunity to provide specialized care to patients with complex heart failure while collaborating with heart failure physicians, APPs, nurses, and multidisciplinary teams. The ideal candidate is passionate about cardiology, enjoys managing complex patients, and values autonomy in a highly collaborative academic environment.



Position Highlights




  • Full-time position with a 4-day, 10- hour weekday schedule
  • No weekends or holidays
  • Outpatient-focused role with a strong emphasis on patient education, care coordination, and improving outcomes
  • Opportunity to work directly with Advanced Heart Failure faculty and participate in clinical research initiatives



Responsibilities




  • Evaluate and manage patients referred to the Heart Failure Bridge Clinic
  • Develop individualized treatment plans and collaborate with physicians on complex cases
  • Assess volume status and determine appropriate interventions, including IV diuretic therapy when indicated
  • Coordinate transitions of care between inpatient and outpatient settings
  • Provide clinical support and guidance to HFBC nursing and support staff
  • Serve as a liaison between heart failure specialty clinics, inpatient teams, and multidisciplinary providers
  • Ensure care delivery aligns with established clinical protocols and quality standards
  • Document and bill for professional services provided



Qualifications



Nurse Practitioners:



  • Adult-Gerontology Acute Care NP (AGACNP) or Adult-Gerontology Primary Care NP (AGPCNP) certification required



Physician Assistants:



  • Graduate of an accredited Physician Assistant program
  • NCCPA certification required



Additional Requirements:



  • Current Maryland NP or PA license (or eligibility)
  • Prior cardiology experience required
  • Heart failure management experience preferred
  • New graduates may be considered with prior cardiology experience.
  • Strong clinical judgment, communication skills, and ability to work collaboratively within a multidisciplinary team

Salary Range: Minimum 58.03/hour - Maximum 89.94/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
