No more than 1 accident or 1 moving violation in a 12-month period, in the last 3 years. No DUI, DWI or DWAI charges in a commercial motor vehicle or in a personal vehicle within the previous seven (7) years. No conviction associated with a recordable DOT accident. No Driver's license suspension within the previous seven (7) years because of a moving violation, failure to pay fines, failure to appear, and/or failure to meet the obligations as required by the state licensing agency to maintain an active CDL.
