Care Coordinator (CLTS) - Ozaukee County
Lutheran Social Services of WI & Upper Michigan

We're Growing! Make a difference in the lives of children and families in your community.

Lutheran Social Services of WI & Upper Michigan is seeking a full-time Care Coordinator to support children and families participating in the Children's Long-Term Support (CLTS) Waiver Program in Ozaukee County. In this role, you will help families access services and resources that enable children with developmental, physical, or severe emotional disabilities to thrive safely at home and in their communities.


What You'll Do

  • Serve as the primary point of contact for children and families enrolled in the CLTS program
  • Complete assessments and identify service needs
  • Develop and coordinate individualized, family-centered service plans
  • Connect families with community resources and providers
  • Facilitate team meetings and collaborate with service partners
  • Maintain accurate documentation and client records
  • Provide trauma-informed, person-centered support

Qualifications

  • Bachelor's degree in Social Work or a related Human Services field
  • At least one year of experience working with children and/or youth with disabilities, or an equivalent combination of education and experience
  • Valid driver's license and reliable transportation
  • Strong communication, organization, and documentation skills
