Population Health Services Organization helps support UC San Diego Health's Primary Care practices and is part of the team-based model of care. Our department supports patient outreach focusing on areas from wellness to end of life across the lifespan. Our goal is to ensure timely care connections, help with health education, and inform patients about our model of right care, right place, right time. Our mission is to support the Quadruple AIM: quality, cost, patient and team experience/wellbeing. This is an exciting opportunity for a nurse with demonstrated nursing education interest and process improvement knowledge to grow with us as we build out our programs helping all patients, especially vulnerable populations across all ages who need additional help to better manage their health and safety at home.Applies skills and experience as a professional nurse educator to ensure optimal clinician knowledge and skills through education and training in either general or specialized nursing education and/or training. Leads implementation of new programs as it relates to student expansion. This position is on site Monday - Friday with students. During student/ university breaks, this position will be able to work from home as appropriate. Key Responsibilities:

Pay Transparency Act Annual Full Pay Range: $123,000 - $237,800 (will be prorated if the appointment percentage is less than 100%) Hourly Equivalent: $58.91 - $113.89 Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable). | |
