Practice at the top of your license while making a meaningful impact in the lives of medically complex patients. Optum's Senior Community Care team is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care in skilled nursing facilities throughout Lane County.

As the primary Advanced Practice Clinician for your patient panel, you'll have the autonomy to make clinical decisions, manage your schedule, and build long-term patient relationships, all within a value-based care model that prioritizes quality outcomes over productivity metrics. Position Highlights & Primary Responsibilities
  • Conduct comprehensive patient assessments
  • Manage acute and chronic conditions, diagnostics, and treatment plans
  • Provide care during sub-acute rehabilitation episodes
  • Lead goals-of-care, advance care planning, and advanced illness discussions
  • Collaborate with caregivers, facility staff, PCPs, and interdisciplinary teams
  • Educate patients and caregivers
  • Maintain accurate clinical documentation
About the Patient PopulationCare for high-risk Medicare patients with multiple chronic conditions, advanced illness, and complex medical, behavioral, functional, and social needs. Focus on improving outcomes and quality of life through personalized, relationship-based care. Required Qualifications Nurse Practitioners
  • MSN or DNP from an accredited program
  • Active NP license (or ability to obtain by start date)
  • AANP or ANCC certification in an approved NP specialty
Physician Assistants
  • Graduate of an accredited PA program
  • Active PA license (or ability to obtain by start date)
  • NCCPA certification
Additional Requirements
  • DEA licensure/prescriptive authority (or ability to obtain post-hire, where applicable)
  • Ability to obtain a collaborative practice agreement if required by state law
  • Valid driver's license and reliable transportation
  • Ability to meet the physical requirements of a field-based role
Preferred Qualifications
  • 1+ years of clinical experience, preferably in long-term care
  • Experience managing patients with complex medical, behavioral, social, or functional needs
  • Knowledge of geriatrics, chronic disease management, advanced illness, and end-of-life care
  • EMR and clinical documentation proficiency

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
