Advanced Practice Clinician (NP/PA) - Senior Community Care
Optum | Washington & Multnomah Counties, OR | Full-Time | Field-Based Practice at the top of your license with the autonomy to make clinical decisions, manage your own schedule, and drive patient outcomes. Optum's Senior Community Care team is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care to medically complex patients in skilled nursing facilities throughout Washington and Multnomah Counties. As the primary Advanced Practice Clinician for your patient panel, you will serve as the key clinical decision-maker, partnering with PCPs and an interdisciplinary care team while maintaining ownership of your patients' care plans. Optum's value-based model prioritizes quality outcomes over patient volume, allowing you to focus on delivering exceptional care without productivity quotas or RVU requirements. Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
