Description

Under the direction of the ID Program Manager, the HIV Prevention Navigator provides comprehensive, patient-centered navigation services that support individuals who are eligible for or currently receiving HIV Pre-Exposure Prophylaxis (PrEP). The Navigator serves as a key member of the HIV prevention program within the Infectious Disease (ID) team, ensuring timely linkage to PrEP services, retention in care, medication access, adherence support, and ongoing patient engagement. This position works closely with medical providers, the pharmacy team, community health workers, and insurance payers to reduce barriers to HIV prevention services, and provides education and navigation for all PrEP modalities, including oral and long-acting injectable PrEP medications, while ensuring patients receive assistance with insurance coverage, prior authorizations, pharmaceutical assistance programs, and benefits coordination. The ideal candidate demonstrates cultural humility, strong organizational and communication skills, is persistent and resourceful, and shows a commitment to advancing health equity for populations disproportionately impacted by HIV.

Representative Duties:

Provides Patient Engagement and Education

  • Conducts outreach and engages individuals who may benefit from PrEP services.
  • Provides education regarding HIV prevention strategies, PrEP eligibility, effectiveness, side effects, adherence requirements, and available prevention options.
  • Educates patients about oral and long-acting injectable PrEP therapies.
  • Supports informed decision making through culturally responsive communication.
  • Provides education regarding nPEP (non-occupational Post-Exposure Prophylaxis) and facilitates linkage to appropriate services when indicated.

Coordinates PrEP Navigation and Care

  • Guides patients through the PrEP care continuum from referral and enrollment through initiation, adherence, and retention in care.
  • Coordinates new patient intake, risk assessments, and PrEP eligibility screening.
  • Schedules medical appointments, laboratory visits, and follow up appointments.
  • Facilitates referrals to primary care, behavioral health, substance use treatment, social services, housing resources, and community programs as needed.
  • Conducts follow up outreach to maintain engagement and reduce missed visits.
  • Maintains ongoing communication with providers and multidisciplinary team members regarding patient needs and barriers to care.

Supports Insurance, Benefits, and Medication Access

  • Verifies insurance eligibility and benefits coverage.
  • Completes and submits prior authorization requests for PrEP medications.
  • Monitors authorization status and coordinates appeals when required.
  • Assists patients with pharmaceutical copay assistance programs, patient assistance programs, and financial assistance resources.
  • Coordinates with pharmacy and clinical teams to facilitate medication acquisition and administration.
  • Educates patients regarding coverage options and out-of-pocket cost reduction opportunities.
  • Resolves medication access barriers to ensure timely initiation and continuation of PrEP therapy.

Supports Long-Acting Injectable PrEP Navigation

  • Coordinates scheduling for injectable PrEP administration visits.
  • Tracks injection due dates and follow up intervals, and assists patients who miss appointments with rapid re-engagement and rescheduling.
  • Collaborates with providers, pharmacy staff, and nursing teams to support adherence to long-acting injectable PrEP schedules.
  • Monitors retention and continuity of care for patients receiving injectable PrEP.

Supports Digital Navigation and Patient Technology Access

  • Assists patients with accessing and using the patient portal (MyChart) for scheduling, messaging, and reviewing PrEP-related results and reminders.
  • Supports patients in preparing for and troubleshooting telehealth visits related to PrEP initiation, follow up, and monitoring.
  • Helps patients evaluate health information they encounter online or through AI tools, and redirects them to accurate, evidence-based PrEP resources.
  • Completes Fenway's Digital Navigation training and applies these skills in day to day patient support.

Supports Ryan White Program Compliance

  • Completes Ryan White HIV/AIDS Program grant-funded staff training as required, and maintains working knowledge of Ryan White eligibility, reporting, and documentation requirements relevant to this role.
  • Documents patient encounters and services in alignment with Ryan White reporting timelines and requirements, in partnership with the ID Program Manager.
  • Supports accurate and timely data collection for Ryan White-funded HIV prevention and PrEP navigation services.

Maintains Documentation and Data Management

  • Maintains accurate, complete, and timely documentation within the electronic health record (EHR).
  • Ensures compliance with HIPAA, organizational policies, and funding requirements.

Participates in Community Outreach and Health Equity Initiatives

  • Participates in community events, health fairs, educational workshops, and outreach activities.
  • Develops and maintains relationships with community partners to enhance referral networks.
  • Promotes equitable access to HIV prevention services among disproportionately impacted populations, including cis women and cis women of color.
  • Supports initiatives addressing social determinants of health that affect PrEP access and adherence.

Meets Agency Participatory Expectations

  • Participates in team huddles, case reviews, and collaborative care planning with the ID and Sexual and Reproductive Health teams.
  • Participates in department and health center quality improvement activities as requested.
  • Adheres to the highest principles of patient and client confidentiality.
  • Attends required meetings, in-services, and professional trainings.

Requirements:

  • Bachelor's Degree (Preferably in a health care related field)
  • Knowledge of HIV prevention strategies and PrEP services.
  • Experience working with diverse and underserved populations.
  • Proven task management and strong documentation habits.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to build trust with vulnerable populations.
  • Initiative and independence, with the ability to manage multiple priorities and maintain confidentiality.
  • Proficiency in Microsoft Office and electronic health record systems.
  • Completion of Ryan White HIV/AIDS Program grant-funded staff training within 30 days of hire, and annually thereafter.
  • Completion of Fenway's Digital Navigation training within 90 days of hire.
  • Completion of the HealthHIV PrEP Navigation Certification (HIVPNC), provided by Fenway, within 90 days of hire.
  • Demonstrated ability to work respectfully and effectively with individuals from a wide range of backgrounds, perspectives, and life experiences, without regard to race, color, national origin, sex, gender identity, sexual orientation, religion, age, disability, or other protected characteristics as defined by applicable law.

Preferred Qualifications:

  • Experience in healthcare navigation, case management, patient advocacy, HIV prevention, or related services.
  • Experience with insurance verification, prior authorizations, and pharmacy benefit management.
  • Experience supporting patients receiving long-acting injectable therapies.
  • Bilingual or multilingual proficiency.
  • Familiarity with Ryan White, Medicaid, Medicare, commercial insurance, and patient assistance programs.
  • Experience working within infectious disease, sexual health, LGBTQ+, community health, or population health settings.
  • Experience supporting patients with digital health tools, patient portals, or telehealth workflows.

Physical Requirements:

  • Ability to meet the following physical requirements with or without reasonable accommodations:
  • Physical work environment is generally that typical of an office or outpatient clinic setting in terms of lighting, temperature, and structure.
  • Ability to sit at a computer station and keyboard for extended periods of time.
  • Some local travel between Fenway sites and community locations for outreach and events.
  • May have periodic in-person contact with patients in clinical settings, requiring adherence to standard infection control precautions.
