Scope of Position/Position Summary: Social and Behavioral Health services patients, families, students, and the community to meet their wellness, social and emotional needs.
Social Work improves patients' lives by providing services to meet their wellness, social, and health needs. Helps individuals deal with a variety of mental health and daily living problems to improve overall functioning. Social work services include psychosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, health education, and the provision of material resources and linkage with community agencies. Works to ensure a safe transition from one care environment to another.
Social work services include psychosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, health education, and the provision of material resources and linkage with community agencies. Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact their medical condition, treatment, recovery, and safe transition from one care environment to another. Core competencies include: Psychosocial assessment, counseling, and crisis intervention skills, knowledge of post-acute levels of care and the PASAAR process for extended care facility placement, assessment and counseling processes involved, and skills assessing neglect/abuse, including substance abuse and domestic violence. Minimum Qualifications: For Hire: Master of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work licensure (LSW)
