Job Summary Utilizes specialized knowledge and skills within major areas of focus, such as patient / family, nurses and nursing practice, and the organization / system. Provides specialized clinical education in regards to treatment and liaisons with the patients clinical service providers to address specific concerns. Has in-depth specialty expertise and focus.
This Nurse Coordinator role is part of a dynamic care team within the Office of Population Health (OPH) Clinical Programs Relay Center. This position will report to the Manager, Transitions of Care. The Nurse Coordinator is a hybrid remote position with some requirement for travel to various UCSF Locations or the Brisbane Office to meet Divisional priorities. The incumbent should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.
The Nurse Coordinator assists with navigating all aspects of the healthcare system. The Relay Center is charged with reporting Laboratory, Microbiology, and Radiology test results to patients or internal/external providers who ordered tests that were performed at UCSF. This could include test results from the Emergency Department, Inpatient or the Ambulatory care settings. The Nurse Coordinator collaborates with various internal and external partners to ensure that patient results are handled appropriately and according to standard procedure. This position may involve coordinating care for patients without primary care providers, assisting with referrals and scheduling follow-up testing, and facilitating connections to community resources. The Nurse Coordinator is responsible for handling clinical escalations from the team's Health Care Navigator(s). The Relay Center team works to meet patient expectations, achieve quality outcomes, and build a patient-focused culture. In addition, all Relay Center staff work with leadership to develop workflows and standard operating procedures, assist with quality improvement projects, and track key performance metrics and outcomes. The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $148,800 - $223,200 (Annual Rate).

Required Qualifications
- BSN Graduate from an accredited school of nursing
- Minimum of two years of recent experience as a registered nurse caring patients in an acute care setting
- Current CA license to practice as a Registered Nurse
- AHA Basic Life Support (BLS) certification
- Strong knowledge of medical terminology
- Proficiency with EPIC APeX applications
- Excellent written and verbal communication skills
- Proficiency with Windows-based software including Microsoft Word, Excel, Outlook, Teams
- Ability to work independently in a fast-paced, demanding environment with minimal supervision
- Ability to manage and oversee multiple tasks simultaneously, prioritizing and adjusting as needs change
Preferred Qualifications
- Experience working with test results
- Knowledge of data collection, compilation, and analytical techniques.
- Experience developing and implementing workflows and standard procedures to improve operations.
- Bilingual/Multilingual
- Experience working with both pediatric and adult patients
- Experience working as a registered nurse in an Emergency Department
- Experience with telephone triage
License/Certification
- Current CA license to practice as a Registered Nurse
- AHA Basic Life Support (BLS) certification
