Pediatric General Surgeon NEXE

Provider agrees to:

1. Notify Children's on a timely basis of Provider's inability to provide coverage to due vacation, leave, illness or otherwise.

2. Provide services to all patients regardless of ability to pay or source or type of medical coverage in accordance with Children's policies and procedures.

3. Cooperate in the timely completion of documents and forms as are required by government agencies and other entities external to Children's for the documentation of duties performed and compensation provided under the terms of this Agreement.

4. Perform such other services as are reasonably requested by Children's ancillary to the foregoing. Provider agrees to:
